How do I refer a patient?
Referrals and Mental Health Care Plans can be faxed to 02 9171 6515 or emailed to reception@tst.clinic. Please address it to TST Clinic, PO box 186, Pennant Hills, NSW, 1715
What needs to be included in the referral?
- Patients full name, date of birth, address, Medicare number and contact details
- Detailed reason for referral
- Sufficient clinical details and relevant medical and family history
- ECG and blood pressure results
